A completed nomination

and signed Tibra Awards Agreement

Nomination letter. The nomination
letter is very important in providing the judges with a meaningful
summary of the nominee's credentials. The nominator can be
a family member, friend, teacher or any person who knows the
nominee and believes she qualifies for the Tibra Awards. The
nomination letter should help the judges appreciate the value
of the nominee's credentials.

A personal statement by the nominee,
which might be ultimately published on the Tibra web site.

Two verifiable references. Letters
written by a teacher, coach, supervisor, etc., but not family
members or friends.

Supporting documents. Copies
of the documents listed below must be submitted with the nomination
form:
a) Current academic transcript
b) Certificate of scores on SAT/ACT or similar standardized
tests
c) Letters or certificates of recognition, honors and awards
d) Documentation of other nominee credentials

A resume of the nominee (optional)

A photo of the nominee that might
be published (optional)

In addition to the above, it
is essential that you send copies of the following documents
in electronic form, either by e-mail to team@tibra.org,
or on a disk with the nomination package:
a) Personal Statement
b) Nomination letter
c) Resume, if included in the nomination package
d) Supporting documents included in the nomination package;
such as essays and writings by the nominee.
It is the responsibility
of the nominee and nominator to put the nomination package together
and submit it to Tibra Foundation before the deadline.
A nomination package
is not considered complete if any of the above requirements is
missing. Only complete packages of eligible nominees will be processed.
Packages that
are not completed or still missing items from the list of requirements
by February 1, 2008 will be considered incomplete and will not
qualify for consideration.
Tibra Foundation
has limited financial and human resources. Please help Tibra maintain
low costs and efficient use of its resources by providing a complete
nomination package in a timely manner.
